Makerspace request form

2. To reserve Makerspace and equipment you must have:

   • A completed Makerspace Waiver and follow the Makerspace Guidelines.
   • A Canon City Public Library card in good standing.
   • Valid photo ID (e.g. Colorado ID, Driver's License, Student ID, etc.)
   • Be age 14 and older. (Users under age 14 must be accompanied by a parent or guardian.)

4. *This form is intended for the reservations for Makerspace and equipment.

   • For teachers interested in Makerspace tours or activities, contact library staff for more information.
   • To book Makerspace staff assistance with your project, complete the Book an Appointment form.

   Notes:

   • Reservations must be made up to seven days in advance.
   • Makerspaces close 30 minutes before the library.
